Terms & Conditions

Welcome to PMA Med Spa. By enrolling in our membership programs, you agree to the
following terms and conditions. These terms are designed to ensure a clear understanding of our services and to maintain the integrity of our offerings.

1. Membership Overview
PMA Med Spa offers various membership tiers, including Bronze, Silver, Gold, and HydraFacial Memberships. Each membership provides exclusive benefits and services tailored to our clients' needs.

2. Membership Commitment

Initial Term: All memberships require a minimum commitment of three (3) consecutive months.

Auto-Renewal: After the initial term, memberships will automatically renew on a month-to-month basis.

Cancellation: Post the initial term, members may cancel their membership at any time.

3. Prescription-Based Treatments

Eligibility: Certain treatments, such as hormone replacement therapy and weight loss

medications, are prescription-based and require active membership status.

Membership Requirement: To receive and continue these treatments, members must
-
maintain an active membership.

Non-Compliance: Obtaining a prescription and subsequently canceling the membership before the end of the prescribed treatment period is considered a breach of this agreement.

Consequences: Such actions may result in termination of services and prohibition from future treatments at PMA Med Spa.

4. Payment Terms

Billing: Membership fees are billed monthly on the date of enrollment.

Payment Method: A valid credit or debit card must be on file for automatic payments.

Failed Payments: If a payment fails, members will be notified and must rectify the issue within seven (7) days to avoid suspension of services.

5. Service Usage and Rollover
-
Monthly Benefits: Membership benefits are allocated monthly.

Rollover Policy: Unused services do not roll over to the next month unless specified in the membership agreement. For example, the Bronze/Silver/Gold membership DO rollover. All money accumulated is therefore banked and can be used on any services at PMA MED SPA (independent contractor and other business withing PMA MED SPA are NOT included).

6. Appointment Policies

Scheduling: Appointments should be scheduled in advance to ensure availability.

Cancellations: A 24-hour notice is required for appointment cancellations.

No-Shows: Failure to show up for a scheduled appointment without prior notice may

result in a $100 fee.

7. Termination of Membership

By Member: Members may terminate their membership after the initial 3 month term for SPA MEMBERSHIPS ONLY – NOT PRESCRIPTION BASED MEMBERSHIPS.

By PMA Med Spa: We reserve the right to terminate a membership for reasons including, but not limited to, violation of terms, inappropriate behavior, or non-
compliance with treatment protocols.

8. Refund Policy

Membership Fees: All membership fees are non-refundable.

Services Rendered: No refunds will be issued for services already rendered.

9. Changes to Terms
PMA Med Spa reserves the right to modify these terms and conditions at any time. Members will be notified of any significant changes.
